Volunteer Application

In addition to filling out this application, you must

Commit to at least one hour per week during the school day for an entire school year.
Attend an orientation. Register by selecting an orientation date on the online application form.
Submit two completed reference forms. References may be personal or professional, but cannot include family members. Bring your completed references to orientation.
Provide proof of a negative TB test result taken after July 1, 2007. Bring your TB test results to orientation. Please note if your test results return positive due to previous immunization we just need a note from your doctor stating that you do not currently have TB.
Provide a photo ID (driver’s license, passport, or state ID) – not a photocopy. Bring your ID to orientation.
Get fingerprinted. We will scan your fingerprints at orientation to do a criminal background check.
