School Volunteer Application (Youth)

Note: This is the Youth application. Adults should use this form to apply.


In addition to filling out this application, you must

  1. Attend an orientation. Available dates are on this application.
  2. Provide the following documents:
    • Proof of a negative TB test result taken after July 1, 2007. Bring your TB test results to orientation. Please note if your test results return positive due to previous immunization we just need a note from your doctor stating that you do not currently have TB.
    • Two completed reference forms. References may be personal or professional, but cannot include family members. Bring your completed references to orientation.
    • A photo ID (driver’s license, passport, or state ID) – not a photocopy. Bring your ID to orientation.
  3. Get fingerprinted.
    • We will scan your fingerprints at orientation to do a criminal background check.
    • Fingerprinting costs us $66 per person. We are asking each volunteer for a one-time suggested contribution of $66 to cover that cost. No one will be turned away due to lack of funds. We will collect the donation at orientation. We will accept payment via cash, check or credit card. Donations are tax deductible.
